VISA FORMS

Applicants are requested to download the visa application forms from the link provided below.  The form is available in Acrobat Reader format.  Applicants whose computers do not have the Reader may visit www.adobe.com for free download.

The form when downloaded should be completed in BLOCK CAPITALS and in English before printing out the quadruplicate copies for signature and submission at the Embassy.  Completed forms should be submitted by applicants in person, together with four (4) recently taken colour passport size photographs, evidence of flight reservation and national passport valid for at least six months at the Consular Section of the Embassy between 10.00hrs and 12.00hrs, from Monday to Thursday.

It is recommended that applicants fill in and print out application forms, as provided.  Hard copies would soon be unavailable at the Embassy for collection and completion by hand.
